Service Agreement

Purpose

Use this LiveVault Service Agreement form to:

View the LiveVault Online Backup Service full service level agreement.
Accept the agreement and to start using LiveVault Online Backup Service.
Not accept the agreement.

This agreement is displayed the first time that you or the first user for your company logs in. A user for your company account must accept this agreement before any user for your company can start to use LiveVault Online Backup Service.

If you do not accept the agreement, you will be automatically logged off. When you log in the next time (or until another user accepts the agreement), you will again have the opportunity to review and accept the agreement. Once accepted, this agreement will not be displayed to any users when they log in for future sessions.

After this agreement has been accepted, it is available for review from MyLiveVault through a link at the bottom of each page.

Fields and Buttons Descriptions

Field/Button	Description
I represent that I have the authority to bind <MyCompany> with respect to this Agreement...	Select this check box to indicate that you (or the person specified in the following fields) have the authority to accept this agreement.
Your Name	Specify the full name of the authorizing person. This defaults to your name (as the logged in user). Changing this field does not change your user account information.
Your Email	Specify the email address of the authorizing person. This defaults to your email address (which you used to log in). Changing this field does not change your user account information.
Your Employer	Specify the name of the company of the authorizing person. This defaults to your company name (as configured for your user account). Changing this field does not change your user account information.
Accept button	Click to accept the agreement. You will receive an email confirming your acceptance of the agreement.
Reject button	Click to indicate that you do not accept the terms of this agreement. The Login to LiveVault Online Backup Service form opens again. You can log in at a later time and view this agreement again. However, you (or another authorized user for your company) must accept this agreement in order for any users to proceed and use LiveVault Online Backup Service.
